We accept most medical insurance plans, including all Medicaid Salud programs. We do not accept any vision plans, but you may file claims with your vision plan yourself. We will help you file your claim if needed.

Remember that many insurance plans require a referral from your primary care provider before we can see you. If you do need a referral, you must get it before your visit, or we will have to reschedule your appointment.

If you have a medical diagnosis, we will file a claim for our services with your medical insurance. If you are not sure we are a participating provider with your insurance carrier, please check with them before your visit. It is our policy to collect payments for non-covered services, deductibles, and co-payments at the time of service and we collect payments for contact lenses when ordered. If you need to make special payment arrangements, please contact us before your visit.

Please bring the following items to your appointment:

  • Current eye glasses
  • Current insurance card
  • Written referral (this includes Medicaid Salud programs)
  • Payment for your insurance co-payment and deductible
  • List of current medications, including eye drops, vitamins, and over the counter medications

If we billed your insurance for a claim, we will bill you for any balance due if you did not pay at the time of service. Accounts billed must be paid in full within 30 days. If you need to make special payment arrangements, please contact our business office.
